What personally identifiable information is being collected?
When you visit the Born Midwives’ website, you have the option of filling out an online form regarding your self-referral. The form is a secure form through the Ocean by CognisantMD platform, the industry’s leading platform for healthcare providers in Ontario. You are asked to provide us with personally identifiable information such as your name, email, date of birth, health card number, address, phone number, medical history, and pregnancy information. We will store a record of your inquiry along with your e-mail address. In addition, our web servers track various technical details about your visit, such as your computer’s IP address and which pages you visited, however, that information does not typically contain any personally identifiable information about you.
How is the information being used?
The sole purpose of the online inquiry form is to add you to our client list for midwifery care. The personal information given in the online inquiry form provides us with a means of adding you to our client list and contacting you regarding your inquiry.
Client Records:
We have a strong commitment to privacy. Our Midwifery Practice Group is bound by law and professional ethics to safeguard your privacy and the confidentiality of your personal information. This includes:
- Collecting only the information that may be necessary for your care;
- Keeping accurate and up-to-date records;
- Safeguarding the medical records in our possession
- Sharing information with other health-care providers on a “need to know” basis where required for your health care;
- Disclosing information to third parties only with your express consent, or as permitted or required by law; and
- Retaining/destroying records in accordance with the law.
You will be asked to sign a consent form that gives your consent for our collection, use, and disclosure of your personal information for purposes related to your care. You have the right to see your records. You may also obtain copies of your records. Please speak with your midwives if you have any concerns about the accuracy of your records or if you would like to request a copy of your chart. If you would like to discuss our privacy policy in more detail, or have specific questions or complaints about how your information is handled, please speak to your midwives. You may contact our Practice Privacy Officer, Mitra Sadeghipour, with questions or concerns.
All records will be kept confidential. We keep all your original records in a secure manner for 28 years following your baby’s birth as required by law. These are available to you at any time. You can request a copy of your records at your discharge visit if you choose. These records are confidential and are seen only by the health care professionals and their students involved in your care. We are required to maintain statistics of our practice. They are collected for research purposes but do not reveal your identity. In the hospital, standard record forms are kept in the medical records department.
With whom do we share your information?
It is important to note that Born Midwives will never disclose your personal information with third parties. Your personal information can only be accessed by Born Midwives. We will never disclose any personal information to parties outside of Born Midwives without your explicit prior consent with the exception of the following:
The following are the only times at which Born Midwives may disclose your personal information: upon request by a legal authority or in good faith when such action is deemed necessary (a) in order to abide by applicable laws or regulations or to intervene in a lawsuit against Born Midwives; (b) in order to protect or defend the legal or property rights of Born Midwives or those of its users; and (c) to intervene, under extreme circumstances, in order to protect the personal safety of Born Midwives’ employees or the safety of the general public.
What are cookies and how do we use them?
